If your phone or tablet falls out of sync with the actual time, the one-time password will fail. That or the caps lock is on. If you haven't set one up, you do not need to use a one time password. Just leave it blank and just enter your regular password. However I would highly recommend setting one up because hacked accounts happen frequently without them. My account got hacked twice before I started using it for example. Hasn't happened since.
